Confidentiality: We operate under HIPAA (Health Insurance Portability and Accountability Act) guidelines and patient confidentiality is important to us, because of the nature of what we do.  Consequently, your health information will not (and cannot) be shared with an outside person, provider, or agency without your express and documented consent.  However, there are times and circumstances in which exceptions must be made and some of your treatment information might be shared.  This would include times when the clinician’s opinion is that there is a risk of harm to yourself or someone else, and in this case, steps will be taken (as is our legal requirement) to protect said persons.  Additionally, state law mandates that we notify the appropriate authorities in cases of reported physical or sexual abuse.  Finally, if documents are requested by subpoena, they will be provided to the designated court.

Medication Refills: Medications are only refilled during business hours.  Under no circumstances  will medications be refilled after hours, on weekends, or holidays.  You can additionally have your pharmacy fax a refill request to our office, at the following number:

Emergencies: If you are having a medical or mental health emergency, or having thoughts of harming yourself or someone else, you should call 911 or proceed to the closest emergency room if you are able to safely do so.
