Dear Patients:
It is my regret to inform you that effective May 30th, 2020, I will be closing my practice due to relocation out of the area. I will no longer be able to take care of your psychiatric needs after that time. Please note that my last patient care/clinic day will be May 28th. If you are on a controlled medication and do not have an existing appointment scheduled, it is imperative that you be seen for an appointment before that time.
Please arrange to have another psychiatrist take care of you. If you are not acquainted with another psychiatrist, you may use your insurance company’s provider directory or check with your local medical society for a list of psychiatrists who may be taking new patients. Once you have identified a psychiatrist or practice, please contact my office by May 1st to obtain copies of your medical record.
For your convenience, I have attached a Release of Information Authorization form. Please be sure that you fill out the form completely, and provide a fax number so that we are able to send your medical records to your new physician. After we close our doors on May 30th, you may reach us for medical records release via the email address: [email protected].
